WAIVER: I understand and accept any and all risks involved in Martial arts training. It has been explained to me, the safety precautions in which I need to protect myself from any injuries while attending Martial Arts Class. Such as the use of a Mouth Guard, Hand and Foot Protection, Protective Groin Cup for Men. Furthermore it has also been explained to me that if I have any medical problems, I need to disclose that fact to the Instructor(s) of MarquezMMA. If I have any cuts or wounds, I need to cover and bandage them before any and all classes that I attend. I am fully aware that "THERE IS PHYSICAL CONTACT IN ALL CLASSES AND THAT THERE IS A RISK OF INJURY" and I AGREE TO WAIVE ANY CLAIM AND AGREE TO RELEASE MarquezMMA INSTRUCTOR(S)/Agents, and/or anybody that I may be training with FROM ANY CLAIM FOR INJURIES SUSTAINED AS A RESULT OF PARTICIPATION IN THE MARTIAL ARTS AND MARTIAL ARTS CLASSES INCLUDING INJURIES CLAIMED TO HAVE BEEN CAUSED BY THE NEGLIGENCE OF THE ACADEMY AND/OR ITS INSTRUCTORS/AGENTS/STUDENTS. If I feel that any of the techniques shown will endanger me or hinder any medical problems that I have, I have the right not to perform such technique(s)! I also understand that all safety equipment must be worn during all classes and that it is my own responsibility to purchase such equipment. I am aware that all safety equipment is available for purchase at the Academy. I am aware that any of the techniques used in class are NOT to be used for anything other than EXTREME SELF-DEFENSE SITUATIONS outside of the classroom. I have read and agree to waive my rights as stated above and fully understand all of the contents within this agreement.

RULES OF SPARRING: I understand that at all times while sparring my protective gear must be worn, that there is light to moderate contact to the body. I also understand that sparring is only permitted when a Head instructor is present. If I do not obey the rules I will be suspended from the Academy indefinitely, and in no way during this time is my obligation for payment suspended and I agree to make all payments as described on this agreement.

MEDICAL HOLD: Buyers may extend his/her membership term under this Agreement at no additional cost, for a period equal to any disability at the end of the current contract term. Injury, illness or disability must require more than 90 days to recover and a physicians (MD) notice stating the same and/or that you are more than 70% disabled in the event of an accident or the academy will not extend any membership. NO EXCEPTIONS!

*Member & Student = one in the same * Application, Contract & Agreement = one in the same */MarquezMMA. By checking off this Agreement, you are stating that you completely understand that this is a legal document and agree with all the terms and conditions stated above.
